Registration Form
Please provide complete & accurate information. All registrations include 1st month/1st semester tuition.
Semester payments ONLY for class lessons, unless arranged differently at your student's school

Music Class Semester is $224 for registration and includes a music book.
Martial Arts Semester is $256 for registration and includes a martial arts uniform t-shirt.

The following types of registration is allowed with written prior approval by PJ's Services, LLC
​
Music Class is $93/student for registration and includes a music book. Monthly tuition is $68.
In-School Private Music Lessons is $169 for registration and includes 3 music books. Monthly tuition is $109.
In-Home/Online Private Music Lessons is $200 for registration and includes 3 music books. Monthly tuition $140

Martial Arts Class is $109/student for registration and includes a martial arts t-shirt. Monthly tuition is $68.
In-Home/Online Private Martial Arts Lessons is $170 for registration and includes a martial arts t-shirt. Monthly tuition is $140.
Multiple students and multiple programs receive a monthly discount. Registration Forms are processed within 48 hours. You will receive a welcome email confirmation once your registration application & payment has been processed. Thank you for your registration.

Release Waiver:
By submission of the Registration Form, I waive any and all claims for my child against PJ's Services, LLC, and any affiliates, or the facilities at which this program is held, PJ's Instructors and staff members, for loss or damage to personal items or injury that may result directly or indirectly from participation in this activity. PJ's Services, LLC has my permission to photograph and/or videotape my child for promotional, training, and educational purposes. PJ's Services, LLC states that my child will never be referenced by name nor will any information about my child be provided. The images will never be sold, rather will be used exclusively for PJ's Services, LLC promotional, educational, or training purposes only.

By submission of the Registration Form, I also acknowledge all of the following: I have read and agree to all of PJ's Services, LLC Terms & Conditions, Liability, Photograph & Video Waivers that are outlined on the website. I am enrolling my child in PJ's Music Program (Includes: Piano, Guitar, Voice, or Violin) or PJ's Mikio Surai Martial Arts Program. I give permission for my credit card to be charged the registration fee. By submitting this form, I am also signing up for the automatic recurring payments according to the type of program I registered for. I understand that my credit card will be charged on the 1st of each month/semester for the tuition, as well as for any other miscellaneous charges that will be owed. I also agree to give a 30-advanced written cancellation notice to cancel the class. Otherwise, I agree to pay 1-month tuition in lieu of that 30-day advanced written notification.
